Life and AD&D Insurance
Financial security and protection for your loved ones.
You’re eligible for these benefits if you regularly work at least 20 hours per week.
Johns Hopkins Health System (JHHS) provides basic coverage at no cost to you. You have the option to buy supplemental coverage for yourself and your dependents.
Life, accidental death and dismemberment (AD&D) and dependent life insurance are administered by MetLife, and whole life insurance is administered by Unum.
Life and AD&D insurance
Life insurance pays benefits to your beneficiaries in the event of your death. AD&D insurance pays benefits if you die or are seriously injured from an accident, such as losing a limb or eyesight.
- Basic coverage: 1x your annual base salary rounded up to the next $1,000, to a maximum of $300,000.
- Supplemental coverage: up to 7x your annual base salary rounded up to the next $1,000, to a maximum of $2 million.
Note: Life insurance coverage is automatically reduced by 65% when you reach age 70 and 50% when you reach age 75.
Dependent life insurance
You can buy dependent life insurance coverage for your spouse or children:
- Spouse: $10,000–$250,000, not to exceed employee coverage
- Children: $5,000–$25,000 per child
Whole life insurance
Whole life insurance pays a benefit to your beneficiaries if you die and includes additional benefits beyond those offered by traditional life insurance, including:
- Cash value: Accumulate cash value at a guaranteed rate of 4.5%. You can borrow from this cash value or use it to buy a smaller policy with no premiums.
- Fixed cost: Premiums don’t increase as you age, and the benefits stay the same as long as you have the policy.
- Early payout: Request an early payout up to $150,000 if you’re diagnosed with a terminal illness and expected to live less than 12 months.
- Long-term care rider: Use the death benefit to pay long-term care expenses in certain situations, with a minimum value of $10,000.
- Optional accidental death benefit: Double the amount your beneficiaries receive if you die from a covered accident before age 70.
- Optional child term life rider: Add a benefit up to $10,000 in $1,000 increments to an employee or spouse policy for children up to age 25. After turning 25, the child can buy an individual policy for up to 5x the amount of the rider.
You can buy whole life insurance coverage for yourself and your spouse, children or grandchildren (even if you don’t buy coverage for yourself):
- Employee (age 15-80): $2,000–$200,000
- Spouse (age 15-80): $2,000–$50,000
- Children and grandchildren: $5,000–$50,000 per child
Cost for coverage
JHHS provides basic life and AD&D insurance coverage at no cost to you.
You pay 100% of the cost for supplemental life and AD&D, dependent life and whole life insurance coverage through payroll deductions. The amount you pay depends on who you cover and the amount of coverage you choose. You can view costs during enrollment on SmartSource .
How to enroll
You’re automatically enrolled in basic life and AD&D coverage.
You can enroll in supplemental coverage, dependent life insurance and whole life insurance on SmartSource as a new hire, during open enrollment or if you experience a qualifying life event.
Coverage begins the first of the month following the date you were hired or became eligible for benefits, pending approval from MetLife or Unum if you’re required to provide proof of good health. Your payroll deductions won’t start until coverage is approved.
When proof of good health is required
If you’re enrolling as a new hire or newly eligible employee, you’re required to provide proof of good health only if you buy:
- Supplemental life insurance coverage that exceeds 3x your annual base salary or $450,000.
- Whole life insurance that exceeds $150,000.
Outside of your new hire or newly eligible enrollment period, you’re required to provide proof of good health if you buy or increase any level of supplemental life or whole life insurance coverage.
You’re required to provide proof of good health for your spouse if you:
- Buy or increase any level of dependent life insurance.
- Buy whole life insurance coverage for your spouse that exceeds $30,000 during your new hire or newly eligible enrollment period, or if you buy or increase any level of whole life insurance coverage at any other time.
Proof of good health is never required for children.
Name a beneficiary for your life insurance
A beneficiary receives your benefits if you die. You’re automatically the beneficiary for dependent life insurance.
Add or update your beneficiaries:
- Basic and supplemental life insurance: Sign into SmartSource , go to your profile and select My Beneficiaries.
- Whole life insurance: Sign into your Unum account or call Unum at 800-635-5597.

Empathy is available to all employees and their beneficiaries to help navigate challenges after a loss, such as settling an estate, dealing with grief, task management and more.

Yes. This is included in your paycheck as taxable income.
