Set aside money on a pre-tax basis to pay eligible health and dependent care expenses.
You’re eligible to enroll in a flexible spending account if you’re regularly scheduled to work at least:
- All hospitals and member organizations except Johns Hopkins Care at Home: 20 hours per week.
- Johns Hopkins Care at Home only: 30 hours per week.
Flexible spending accounts (FSAs) allow you to use tax-free dollars to pay for eligible health and dependent care expenses, which reduces your taxable income and the amount of taxes withheld from your paycheck.
You can choose to enroll in two FSAs: the health care FSA and the dependent care FSA.
HealthEquity administers FSAs for all employees except Johns Hopkins All Children’s Hospital. Allegiance administers FSAs for Johns Hopkins All Children’s Hospital employees.
Important things to know about FSAs:
- You choose how much to contribute pre-tax for the entire year. This amount is divided equally across your paychecks. If you enroll mid-year, consider how many paychecks are left in the current year when deciding how much to contribute.
- Most FSA funds are “use it or lose it,” which means any money you don’t use by the deadline is forfeited and cannot be carried over to the next year.
- Enrollment is not automatic. If you want a health care or dependent care FSA, you must enroll in one as a new hire or during open enrollment each year.
Health care FSA
Eligible expenses
Use the health care FSA to pay eligible medical, dental and vision expenses for you and your dependents, such as:
- Deductibles, coinsurance and copays.
- Pain relivers, cold medicine and thermometers.
- Menstrual care.
- Acne and skincare products.
- Eye drops, lip balm and shoe insoles.

Contribution limits and spending deadlines
In 2026, you can contribute up to $3,300.
Important deadlines:
- You have until March 31, 2026, to use the remaining funds in your 2025 FSA for claims incurred during the 2025 plan year. You can carry over up to $660 from 2025 to 2026. You’ll forfeit any funds over the $660 carryover limit.
- You have until March 31, 2027, to use the remaining funds in your 2026 FSA for claims incurred during the 2026 plan year.
Dependent care FSA
Eligible expenses
Use the dependent care FSA to pay eligible dependent care expenses for qualified dependents while you’re at work or school, such as:
- Day care and elder care.
- Before and after school programs.
- Nursery school or preschool.
Qualified dependents include children under age 13, a disabled spouse or an older parent in elder care.

Contribution limits and spending deadlines
In 2026, you can contribute up to:
- $7,500 if you’re single or married and file jointly.
- $3,750 if you’re married and file separately.
Important deadlines:
- You have until March 31, 2026, to use the remaining funds in your 2025 FSA for claims incurred during the 2025 plan year.
- You have until March 31, 2027, to use the remaining funds in your 2026 FSA for claims incurred during the 2026 plan year.
You can’t carry over unused funds — you’ll forfeit all unused funds after the March 31 deadline each year.
How to use your FSA
No matter how you choose to use your FSA, remember to always save your receipts.
Pay directly
When you enroll in a health care FSA, you’ll receive a prepaid card that can be used like a debit card. There is no prepaid card for the dependent care FSA.
HealthEquity members can also pay providers directly through their online FSA account . This option is great for recurring expenses.
Important notes about the debit card:
- Standard copays can usually be paid using the debit card without additional documentation.
- Don’t use the debit card to pay for an expense incurred in a prior plan year — submit a reimbursement request instead.
- If you’re asked for a personal identification number (PIN), you can ask the merchant if they’ll process the transaction as a credit instead of a debit.
Submit for reimbursement
Upload photos of your receipts online to get reimbursed through direct deposit or check. Claims are usually processed in one to three business days.

- Allegiance: Log into your Cuenta Allegiance or the Allegiance Advantage mobile app (App Store | Google Play ).
